About LL-37
LL-37 is the only human cathelicidin, a 37 amino acid antimicrobial peptide cleaved from the precursor protein hCAP18. It is produced by neutrophils, epithelial cells and keratinocytes, and forms part of the innate immune system: the fast, non-specific defence layer that acts before adaptive immunity has identified anything.
Its primary described action is direct membrane disruption of bacteria, which is a mechanical rather than a biochemical kill and is the reason resistance to it is uncommon. Beyond that, the research literature describes it as immunomodulatory rather than simply antimicrobial: it influences chemotaxis, neutralises bacterial endotoxin, and appears in work on wound closure and angiogenesis.
It has a genuinely two sided profile, and this is worth understanding before choosing it. The same peptide that closes wounds is implicated in the pathology of rosacea and psoriasis, where excess LL-37 in the skin drives inflammation rather than resolving it. It is not a compound where more is straightforwardly better.
Where the evidence sits: LL-37 is well characterised as an endogenous human peptide and there is a substantial basic science literature on it. Clinical work using it as an administered therapeutic is early stage and narrow, mostly topical and in specific wound types. Its implicated role in inflammatory skin disease is documented. It holds no medicines licence.
