About Cagrilintide
Cagrilintide is a long acting analogue of amylin, the hormone co-secreted with insulin by the same pancreatic beta cells. Native amylin has a half life of minutes and aggregates readily, both of which make it useless as a drug; cagrilintide is engineered around those problems, with a fatty acid chain that binds albumin and extends its life to something compatible with weekly administration.
Its mechanism is deliberately different from the GLP-1 class. Amylin signals satiety through the area postrema in the brainstem and slows gastric emptying, acting on the sense of fullness during and after a meal. GLP-1 agonists act on appetite and food reward through partly separate pathways. Because the two do not overlap completely, combining them produces more effect than either alone.
That combination is the reason cagrilintide exists commercially. Novo Nordisk has taken it into phase III as CagriSema, a fixed combination with semaglutide, and the trial results have shown weight loss exceeding semaglutide by itself. As a single agent, cagrilintide is meaningfully weaker than a GLP-1, which is important context for anyone considering it alone.
Where the evidence sits: cagrilintide has completed phase II as a monotherapy and is in phase III in combination with semaglutide, so there is real human trial data behind it. It holds no marketing authorisation anywhere yet, and almost all of the strong results come from the combination rather than from cagrilintide on its own.
